What you'll be doing
- The incumbent practice as a registered nurse under the guidance of the nurse manager for primary health care system and support the mission and vision of the healthcare system. Primary care provide acute and chronic care need for health care system including a wide variety of service for adult and elderly veteran. The pact rural health registered nurse care manager applies pact principles of proactive, personalized, patient centered care, team based care, data driven care, continuity of care coordination, and population health and prevention. The pact rural health registered nurse care manager uses the nursing process and evidenced based practice to collaborate with the team and involved in providing proactive, personalized, and patient centered care to the patient in developing the patient centered holistic plan for life.
What your background should be
- Experience as a registered nurse of an acceptable level of quality with regard to the following four dimensions of nursing; practice, professional development, collaboration and scientific inquiry. United states citizenship required.
Required Schooling / Training
- Graduate from a school of professional nursing.
