What you'll be doing
- Provide a comprehensive assessment of the needs and desires of the elderly client. Prepare a comprehensive care plan for the elderly client to provide to the client and family members. Plan includes action steps and the resources in the community needed to carry out the recommendations in the plan. Assist the client and/or family members with determine eligibility for assistance and resources. Maintain effective communication among family members, resources and services, and client. Act as a liaison to families and long distance caregivers. Possess knowledge of community resources related to medical issues, housing issues, financial, and social issues for dealing with the client care. Make referrals to other professionals. Monitor plan of care and make adjustments as needed. Provide crisis intervention.
What your background should be
- Must be a registered nurse, licensed clinical social worker, or person with certification in gerontology or masters in social work. Minimum of 5 years experience in the field of geriatric care or case management.
