What you'll be doing
- Identify, evaluate and address behavioral health issues of the population in the practice while administering quality care. Ensure all aspects of disease progression and manifestation are appropriately documented. Attend to the frail elderly and chronically ill experiencing psychiatric co-morbidity that can pose barriers to compliance. See scheduled and walk-in appointments for medical patients with psychiatric comorbidity discharged from the hospital setting in need of transition care. Work with staff to perform medication to develop a detailed home plan to ensure acute episode has resolved and the patient is not readmitted. Provide short-term medical management and strategic psychotherapy to bridge patients discharged form psychiatric inpatient settings to bridge them to outpatient behavioral health services. Participate in utilization review activities and recommends treatment alternatives designed to keep patients healthy or resolve on-going clinical issues. Make recommendations to the utilization management team based on patient care trends and treatment challenges of providers. Communicate all services provided. Discharge plan for continuing care management is developed and communicated to ensure continuity of care. Perform other duties as required consistent with a consultative behavioral health service.
What your background should be
- Pennsylvania licensed as a nurse practitioner. Extensive training and/or experience in outpatient behavioral health performing assessments, medical management and psychotherapy. Board certification as a PMHNP. ACLS certification.
