What your background should be

Minimum 1+ years of previous experience assisting patients through the continuum of care and the transition from hospital to home care or hospice. RN strongly preferred. Current CPR certification preferred. Competent organizational skills; ability to handle stressful situations/deadlines; excellent oral/written communication and interpersonal skills. Must demonstrate the ability to communicate effectively with all members of the health care team.
