Registered Nurse

What you'll be doing: Perform assessment, develop individualized patient care plan, and manage multi-disciplinary care as applicable. Provide skilled nursing care in a patient home as ordered by the attending physician in conjunction with the plan of care. Comply with agency policy and procedure. Promote and maintain an agency environment that complies with federal, state, and local regulation. Submit clear and appropriate documentation in a timely manner and document patient education. Communicate effectively to physician, supervisor, and coworker in a timely manner. Participate in case conference to promote coordination of care. Promote continuity of care with appropriate admission, transfer, and discharge. Maintain a professional appearance. Perform all aspect of patient care.

What your background should be: One year of experience as a registered nurse in an acute care setting, preferably two years of experience. Home care experience desired. May provide a health statement signed by a physician and must show evidence of a current negative tuberculin skin test or X-Ray.
