What you'll be doing

Evaluates, plans, directs and administers programs of occupational therapy upon referral by the physician, providing care for infant child, adolescent, adult and geriatric patients on an intermittent basis in the home. Evaluates the patient functional ability to perform activities of daily living and develops a plan of care in conjunction with the patient physician. Observes, records, and reports to the physician any reaction to treatment and any changes in the patient condition. Maintains communication with clinical staff providing updates and changes in schedules, patient data or needs, and physician orders. Designs, develops, and fits patient with orthotic devices and adaptive equipment as needed and instructs family members, home health aides and other clinical staff in appropriate therapy methods. Evaluates patient plan of care as needed and at least every 62 days, revising the plan to meet patient needs. Incorporates short and long-term goals established in the plan of care into therapy, as evidenced by documentation in visit notes, monthly progress notes, and discharge summaries.

What your background should be

Works in patients homes in various conditions. Personal protective equipment such as gloves, goggles, gowns, and masks are sometimes required due to possible exposure to hazardous chemical or blood and body fluids. Ability to work flexible schedule to meet patients needs. Ability to travel extensively. Some exposure to unpleasant weather.

Required Schooling / Training

Bachelor degree in nursing.
