What you'll be doing
- Provide rehabilitation training and support to Montgomery station consumer, which include group facilitation, individual meeting, outing and medication monitoring, onsite at day program or off site with residential or outreach client. Provide basic case management function. Coordination of service with consumer other provider (psychiatrist, therapist, etc.) by effectively and professionally communicate clinical matter on a monthly basis. Identify consumer need and link consumer with community service. Prepare and monitor individual rehabilitation plan (IRP) and maintain accurate, up to date record or documentation. Also provide addendum as needed. Prepare report and documentation as required in a thorough and timely manner. Provide medication monitoring onsite or offsite as assigned by team leader. Assist in orientation and train new admission and new staff to Montgomery station PRP and RRP. Provide transportation for consumer when deemed necessary for meeting consumer need. Attend provider meeting as a part of the Montgomery station treatment team and represent Montgomery station in a professional manner. Provide coverage on assigned holidays through the year. Perform other duties as assigned by supervisor.
What your background should be
- A current, valid driving license, proof of auto insurance (which meets minimum MD state requirements) and a current driving record are also required for transporting consumers, either in agency vans or a personal vehicle.
Required Schooling / Training
- High school diploma required. BA/BS in human service preferred.
