What you'll be doing

Assist in developing and implementing policy and procedure for the identification of medically related social and emotional need of the resident. Participate in community planning related to the interest of the facility and the service and need of the resident and family. Interview resident or family to obtain social history. Participate in the admission, discharge, etc. in the department. Provide information to resident or family as to Medicare or Medicaid and other financial assistance program. In the absence of the director, assume the authority, responsibility and accountability of directing social service department. Coordinate social service activity with other department. Evaluate social and family information and assist in determining plan for social treatment. Interpret social, psychological and emotional need of the resident or family to the medical staff, attending physician and other resident care team member. Assist in obtaining resource from community social, health and welfare agency. Demonstrate a passion for caring as evidenced by interaction with co worker, resident, family and visitor. Perform all other duties, as assigned.

What your background should be

Must have a minimum, one year supervised social work experience in a health care. Must be able to read, write, speak and understand the English language. Must possess the ability to deal tactfully when personnel, residents, family members, visitors, government agencies or personnel and the general public. Must be able to communicate policies, procedures, regulations, reports, etc. to personnel, residents, family members, visitors and government agencies or personnel. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age related needs of the residents served. Must not pose a direct threat to the health or safety of other individual in the workplace. Must meet the general health requirements set forth by the policies of this facility, which include a mental and physical examination.

Required Schooling / Training

Bachelor degree from an approved school of social work or a bachelor degree in a human service field.
