What you'll be doing
- Follow designated protocols and scripts, if available; provide follow up contact with patient as indicated to ensure compliance with provider recommendations and treatment plan medications, lab or X-ray, specialist visits, PCP visits, dieticians, CDE, etc. Manage designated aspects of the patient care: referrals to specialists, hospitalizations, ER visits, ancillary testing and other enabling services. Consult with physicians and clinical staff and providers regarding the treatment process when necessary. Participate in facilitating patient transitions in the care continuum when possible and appropriate. Anticipate the needs of the patient, helping ensure the necessary documentation and referral authorizations are completed. Assess barriers when patient has not met treatments goals, is not following treatment plan of care, or has not kept important appointments. Perform other duties as assigned.
What your background should be
- Experience in primary care with this population is highly desirable. Advance knowledge of medical terminology. Knowledge of basic nursing principles and protocols. Bilingual may be required at certain facilities or during certain shifts. Computer literate (i.e., be familiar with a pc and basic software, such as Microsoft Windows, Microsoft Word, etc.). Must respond sympathetically and maturely to patient needs. LVN to RN license.
