What you'll be doing

The successful position will be responsible for the QI population management project coordinator. The quality improvement population management project will be including contacting, outreach and follow up of patients that need physical exams, immunizations, and management of chronic issues including asthma, depression, pregnancy, etc. S/he will need to develop and track excel spread sheets, contact patients by phone and mail, and help them to schedule visits or follow ups. S/he will assist in the chronic care management of patients with phone check-ins and care coordination. S/he will need to learn Epic (which is the hospital scheduling system) and work with ISD clinical teams to identify population.

What your background should be

Up to one year of basic secretarial training and six to twelve months of previous experience. Ability to work communicates effectively both orally and in writing and provides empathy in difficult interpersonal situations. Work requires the analytical skills and judgment to interpret data and take the appropriate action in circumstances or situations vary. Computer skills important as well as language skills desirable.
