What you'll be doing

Under minimum supervision, serves as a community based member advocate and resource and a vital part of the integrated team. Provides outreach to locate and/or provide support for disconnected members with special needs. Educates members and is an advocate for vulnerable members, engaging and assisting them in managing their healthcare needs. Provides non clinical paraprofessional duties and is thoroughly familiar with the members community and the resources available. Regularly exercises independent judgment and discretion in assisting members with all aspects of their health and wellness by collaborating with companies staff, community based healthcare, social services and other providers. Supports members access to community resources for housing, food, employment, etc. Assists member to maintain medicaid eligibility and with other financial resources as appropriate. Assists members with building a support system through family, friends, and community (recreation centers, support groups, etc) and faith-based organizations (churches, synagogues etc.). Imparts knowledge and develops critical reasoning to enable shared decision-making. Participates in projects and meetings as assigned. Complys with workplace safety standards. Acts as member advocate.

What your background should be

One or more years of experience working within a defined community or member of a defined community with specialized training. Proven competency in health care basics, community resources, communication (verbal and written), individual and family advocacy, social services, health education and service skills and responsibilities. Valid state driving license. Proof of car insurance. Managed care exposure and knowledge of healthcare systems a plus. Knowledge of hipaa and confidentiality rules a plus. Current chw certification preferred (mandated in ohio).

Required Schooling / Training

High school diploma/GED. Graduate of a 2 or 4 year allied health program preferred.
