Evening Case Manager

What you'll be doing
- Provide home visits to link clients and families to services and assess families needs for employment, housing, mental health services, legal assistance or any services identified by case development team. Maintain complete confidential case files and completes all required data collection reporting forms. Develop, implement and participate in street outreach at schools and some evenings around the community and programming at Juvenile hall to gain participants. Assist in planning, coordination, implementation and evaluation of activities throughout program and in collaboration with other youth services programs to ensure effectiveness, proper utilization and contract compliance. Assist with supervision and provide leadership development to interns, youth workers and volunteers. Design and implement evening activities and curriculum development for participants in accordance to the work plan and mission or vision of the agency in the following areas: educational enrichment, social and life skills development, anger management, conflict mediation, health education, violence prevention, youth leadership, career exploration or job readiness, parent support groups, life skills and recreational activities. Arrange guest speakers, workshop presenters and career mentors within the program. Plan and implement experiential learning field trips and activities, including local college tours and career exposure outings and activities for clients on caseload. Act a liaison with schools, community, public agencies and parents or guardians of participants. Provide safe passage to youth daily and transportation to youth on all outings. Attend regularly scheduled staff, program team meetings, individual supervision meetings, weekly case development sessions and monthly trainings as required. Participate in the planning and production of rites of passages ceremonies for participants. Participate in company events including planning and implementation prior to and the day of the event.
What your background should be
Required Schooling / Training
- Bachelor degree or successfully working towards a BA or BS in a youth related field.
