What you'll be doing

The responsible person interacts with clients in the community and assists in providing crisis services to clients, referral sources, and family members under close supervision of crisis outreach professionals. Completes necessary paperwork as required, meeting established timelines. Follows all procedures for obtaining, securing and returning client files. Follows all procedures for making authorized entries in the medical record and follows all procedures for release of information. Maintains complete, accurate, legible and concise records and reports. Provides translation services (spanish/english) for clients in emotional crisis. Helps clients identify, understand, and combat stigma and discrimination associated with mental illness and develop strategies to reduce self-stigma. Collaborates with the team to help clients learn about pertinent grievance procedures and support clients with filing, mediating, and resolving complaints. Provides work-related supportive services, such as case management, assistance securing necessary clothing and grooming supplies, wake-up calls, transportation services, etc. Assisting clients to plan and carry out leisure time activities on evenings, weekends, and holidays.

What your background should be

At least two years of paid or volunteer work experience with adults with severe and persistent mental illnesses. Must have good oral and written communication skills. Ability to be self-motivated, work independently and as a team member and represent the agencies in a professional manner within the community. Must have valid drivers license access to a safe vehicle and possess the minimum required liability insurance in accordance with agency policy.
