What you'll be doing
- The Case Manager will be asked to provide Case Management directly to the assigned case load. Besides, the incumbent will have to develop and implement the IP (Industrialized Plan), coordinate and monitor the process of implementation of services, and support written in the IP. The chosen candidate will also be required to help individuals and their families in acquiring services. The Case Manager will help connect each family with a variety of resources that are the most important.
