What you'll be doing
- The Clinical Pharmacy Technician will help medication reconciliation processes within the Medical Center. The incumbent will execute medication reconciliation by interviewing patients (and families/caregivers as appropriate) to develop an accurate list of medications, strengths and dosages the patient has been taking, contacts other pharmacies, when necessary, to complete an accurate list, assesses patient compliance with prescribed therapy through patient/caregiver interview and a review of refill dates. This experienced person will review medication profiles for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies / adverse drug reactions. She/he will also report all potential patient or pharmacy distribution problems verbally and/or in writing as requested by the pharmacist in the area assigned.

- 3 months of relevant experience
Required Schooling / Training
- High school diploma
