Case Manager

What you'll be doing: Our Health Care Alliance is on the lookout for a professional Case Manager (preferably bilinguals with fluency in Spanish), who will be required to render case management and direct support services to the people and their families residing at the Emergency Apartment and Emergency Shelter Program. The person taken for this role will render both shelter-based services directly to the select case load and home-based services to the 6 residents of the emergency department. The number one purpose of the position is to help the family to stabilize their situation and perform a successful transition to the self-sufficient and independent life. In addition, the Case Manager will link the family members to the required services and coordinate these services, as needed by the family and required by the program. The most suitable individual will be open to working directly at the shelter.

What your background should be: To succeed in the Case Manager position you must: - Be able to read, speak and write Spanish. - Be able to cope with the tasks independently. - Be able to work in a corporate team, in other words
