What you'll be doing

Provide therapy services planned, delegated and supervised by the qualified Physical Therapist consistent with the patient approved plan of treatment. Observe patients during treatment, compile data and report patient progress to Physical Therapist. Act as a liaison and maintain contact with patients, Physical Therapist, Clinical Manager other members of the healthcare team and the community in assuring effective care coordination and appropriate resource referrals. Recommend to Physical Therapist possible modification to existing physical therapy treatment plan and training need to achieve maximum results. Maintain and submit documents as required by the Company and/or facility. Accurately and thoroughly documents patient care observation, intervention and evaluation on the day services are rendered. Assist qualified Therapist with preparation of patient progress notes. Participate in performance Improvement activities as assigned. Communicate information that enables collection and root cause analysis of data to identify opportunities for improvement. Maintain the highest standard of professional conduct in relation to information that is confidential in nature. Share information only when recipient right to access is clearly established and sharing of such information is in the best interest of the patient. Adhere to and participate in company mandatory HIPPA privacy program/practices and business ethics and compliance programs/practices. Review and adhere to all company policies and procedures and the employee handbook. Participate in special project and perform other duties as assigned.

What your background should be

Current CPR certification is required. The candidate must successfully complete any/all required pre employment evaluation test per policy in each assigned area of practice. Good organizational and communication skills are needed. Reliable transportation is required.
