What you'll be doing
- Continuously reviews current JCAHO standards and implements appropriate programs for compliance. Actively participates in the companys information system development and serves as system contact for operational issues. Recognizes problems, opportunities, threats, etc., and makes appropriate recommendations to prepare and react. Develops and implements a comprehensive plan to help staff continue future development and to constantly take advantage of opportunities. Identifies new programs. Studies and implements if program is profitable or strategically advantageous. Creates an environment of positive growth through actively serving on the company management team. Works in conjunction with the community education staff to develop new innovative programs. Works with the community education staff to promote advanced home care by new program development, operational backup, personal visits, coordination of educational activities, etc.Serves as a resource in assisting care managers and field staff with problem solving in order to provide the most appropriate patient care. Serves as a resource to the care manager and field staff in evaluating needs and implementing plan of care. Monitors patient outcomes regularly through results of surveys, reports, staff meetings and patient interactions and proactively identifies actual and potential problems and initiates action to continually improve patient outcomes. Acts as a resource and has working knowledge of Medicare, Medicaid and private insurance guidelines related to admission/service criteria and their proper documentation. Periodically reviews documentation (SOC, recert, case conference, 90 days, D/C summary) of clinical staff to evaluate for appropriateness of care, skilled services documentation, homebound status and timely submission. Directs process and performance improvement actions in response to compliance, OBQI, or IOP audits and recommendations. Identifies opportunities to improve efficiency of department and directs actions for improvement. Assists in development and/or revision of clinical policies and procedures.
What your background should be
- Candidate must have three years of Home Health Management with a similar sized company. Must have a current licensed from the state in which he/she would be practicing. Excellent verbal and written communication skills, time management and organizational skills, interpersonal and problem-solving skills, and flexibility.
Required Schooling / Training
- Associate degree in Nursing, Social Work or a Rehab Specialty from an accredited or approved school; BS preferred.
