What you'll be doing
- The incumbent is responsible for the clinical, quality and cost effective outcome for a caseload of patient. Facilitate coordination of an interdisciplinary plan of care to guide patient progress toward achieving clinical, quality and cost effective outcome. The care manager mobilizes resource and manages the system in response to patient need according to an interdisciplinary plan of care. The candidate as an expert clinical practitioner, participate in the development and evaluation of clinical guideline or standard of care for a designated population of patient. Collect, trend and analyze aggregate outcome data for that population to be used in identifying best practice and targeting performance improvement effort.
What your background should be
- Three years recent clinical experience in area of specialty preferred. Current certification in specialty field preferred. Case manager certification, highly desirable. Strong interpersonal skill. Effective listening and organizational skill. Attention to detail required. Strong analytical skill. Excellent PC skill. Proven customer service skill. Proven data entry skill. Time management and priority setting skill.
Required Schooling / Training
- Bachelor degree in nursing required. Master degree in nursing preferred.
