What you'll be doing

The Case Management Associate we will hire assists Triage Case Manager and Clinic Case Manager in providing everyday follow-up and case management services as required during the scheduled triage and clinic sessions. The services the Case Management Associate must deal with include transportation assistance, pharmacy copays / pays / PAPs, routine financial assistance applications, insurance program applications, food referrals and other general follow-up as assigned. Besides that, the Case Management Associate assists in multiple program management tasks as required by the Program Manager
