What you'll be doing

We are searching for a key provider of our services. The Case Manager will be required to work with the customers in order to assess their needs and strengths, plan a proper course of action, counsel the customers and track the progress. The chosen person for the Case Manager role will assure that clients are provided with competent, appropriate and professional advice as well as information, advocacy and referrals in order to achieve the best outcomes. Principal duties and responsibilities for the Case Manager position: - Develop with the organization client an agreed-upon individual service plan reflecting the goals of the assessment findings. - Meet with the clients on a regular basis in order to help her / him in obtaining required benefits and services. - Manage the client intake and assessment process in order to orient the customer to the program. - Document all case management activities according to the funder and agency guidelines. - Cope with the other related tasks as required.

What your background should be

We are searching for the candidates, who have Bachelors Degree (or, as an alternative, 60 college credits) and 2 years of previous professional experience. Besides, the chosen person for the Case Manager role must also have a demonstrated ability in written and verbal communication, as well as basic PC skills.
