What you'll be doing

Provide moderately complex case management work. Perform clinical screening, assessment and service eligibility evaluation based on consumer need. Coordinate and link appropriate service and monitor consumer progress with meeting treatment goal. Deliver intervention such as crisis resolution, case management service and discharge planning. Provide clinical service to address the need for on going service and/or inpatient admission. Facilitate multiple care aspect (case coordination, information sharing etc). Provide crisis follow up and relapse prevention service when appropriate. Perform all other duty as assigned.

What your background should be

Preferably have at least three years experience working with mental health consumers in a crisis situation and also experience working with consumers in the home/community environment. Must have basic understanding of substance abuse and related issues. Must have a valid Texas driver's license, a good driving record for the past three years and a valid auto insurance card. Must be able to bend, stoop, kneel, crouch, squat, lift at least 25 pounds, sit for an extended period and reach overhead.

Required Schooling / Training

Graduation from an accredited four years college or university with a major course work required such as human/social services or psychology.
