What you'll be doing

Conducts thorough diagnostic evaluation of psychosocial issues and goals, including risk factors. Identifies unmet needs in patient or family functional, adaptation and resources states. Identifies patient or family need for information and education regarding end of life psychosocial issues. Continuously assesses and identifies changing service needs, reflecting a strengths based approach to social work practice. Presents information about social, emotional, environmental, cognitive, behavioral factors. Actively, consistently assures the development of the interdisciplinary plan of care that addresses psychosocial needs driven by patient or family identified goals. Identifies roles for volunteers and other services, when appropriate. Provides individual and family counseling based on knowledge of psychodynamics, psychopathology, group dynamics, family systems and human growth and developmental needs throughout the life cycle. Offers a variety of treatment modalities as indicated including, but not limited to, the following therapies: solution focused, family, cognitive, insight oriented, guided imagery, reality, narrative and motivational interviewing. Provides grief counseling to individual, families or groups.

What your background should be

The applicant should have experience in related field. Demonstrate effective communication and relationship building skills with patient or family. Demonstrate ability to advocate for patient or family in effective manner. Must have basic communication skills.
