What you'll be doing

The candidate is responsible for reviewing, analysing, and coding medical diagnoses abstracted from clinical documentation. Ensure timely & accurate coding of medical records in compliance with the company regulatory conditions of participation, ICD-9 & ICD-10 coding guidelines, and organizational standard. Assign most appropriate ICD-9-CM/ICD-10-CM codes for company reporting purposes after thorough review of the medical record and queries physician in a professional manner. Ability to code charts without the use of an encoder review clinical documentation against documentation guidelines and billing codes to ensure accurate billing abides by the standard of ethical coding as set forth by the American health information management association (AHIMA) and adheres to official coding guidelines. Maintain continuing education hours annually, or more as necessary. Ability to identify any local coverage determination (LCD), medical necessity issues. Report outstanding issues and potential solution regarding these coding challenge to the manager. Participate in ongoing review of record as needed to promote quality documentation to support appropriate E&M level of billing. Coordinate & conduct education for staff on clinical documentation guideline to support ICD9-CM/ICD10-CM coding and how documentation impact the coding process. Participate in process improvement initiative as needed.

What your background should be

3-5 years direct coding assignment and validation experience in a multidisciplinary environment, hospice coding experience highly desired. Knowledge of medical record documentation guidelines. Depth knowledge of ICD-9-CM, ICD-10-CM, and CPT-4 Coding systems and knowledge of HCPCS Level II coding system. Certification through the American health information management association as a certified coding Specialist, CCS-P or RHIT. Expertise in Microsoft office suite.
