Collaborative Care Program Analyst

What you'll be doing: The collaborative care program analyst will support ccc program development and maintenance for all markets in the region. The candidate will assist in maintaining national ccc tracking tool, preparing for and managing ccc market calls, compiling action items following jocs and tracking with local teams through resolution, tracking and monitoring ccc issues for the region, prepare quarterly cac performance slide deck, and serve as a resource to direct contracting to ccc resources. Maintain national ccc tracking tool, this includes adding new prospect, updating those in negotiation, and adding quarterly performance results and initiative details to the tool. Follow internal team to insure timely closure and on the action escalation and ccc project tracking action items. Draft agenda and supporting reports for all ccc market calls. Take meeting minutes during the call and make necessary updates to the national ccc tracking tool. Track all ccc related issues, concerns, and requests for the region. Prepare quarterly joc performance slide deck. Serve as a resource for contracting and hcp solutions relative to all ccc programs. S/he will direct team to available resources. Generate all ccc related reports as necessary. Pull intake forms from p4p intake box, add to share point, and direct as appropriate. Perform other duties as determined by the payment innovation director.

What your background should be: The candidate should have excellent project management skills, excellent technical skills, and ability to turn information into presentations for meetings. S/he should have excellent communication skills in written and verbal, and excellent working knowledge of microsoft access and excel. The applicant should be comfortable interfacing and communicating with all varying levels of management.
