Hospice on Call RN

What you'll be doing: This is an on call position. The on call hospice RN will provide telephonic triage and visits as needed. The on call RN will provide coverage for the patients in census and will need to be available to travel throughout hospice service area for visits when required. The responsibilities for the hospice on call RN include: providing telephonic triage to hospice patients and their families during on call hours; conducting visits as needed (pronouncements, tuck ins and admissions); working with an interdisciplinary team; providing education and support to patients and families/caregivers; participating in a daily conference call to provide updates on calls received/visits conducted; performing physical activities that require considerable use of your arms and legs and moving your whole body such as climbing, lifting, balancing, walking, stooping, and handling of biohazard material.

What your background should be: RN licensure (current, unrestricted) in the state location of position and 3 years of clinical experience is required. The applicant should have reliable transportation including valid driver license and auto insurance. Excellent communication skills preferred. Home care and/or hospice care experience preferred. Computer proficiency with MS Office products including word and outlook and proficiency with electronic medical records (EMR) software preferred.
