Infusion Coordinator

What you'll be doing: The Infusion Coordinator will promote our products and services to doctors offices, hospitals, and clinical practices to grow our business coupled with Clinical Coordination. Clinical Coordination includes communication and coordination with the clinical pharmacy, contracted nursing agency, IV payer services team, and the patient/family, to ensure prompt and efficient set up of infusion therapy referrals. The infusion coordinator will provide ongoing communication with the patient/family and appropriate care participants. Serves as an expert clinical infusion resource by coaching, role modeling, and by using educational strategies to pharmacy staff, contracted nursing agency staff, patients/family, referral sources and the community. Participates in the development and marketing of the clinical infusion program to referral sources in cooperation with the sales and marketing team thereby contributing to the achievement of corporate objectives/goal. Facilitates the transition of patients from hospital to contracted nursing agency to include (but not limited to) pre-discharge set-up and teaching in the hospital and coordination/direct communication with hospital case managers/discharge planners, contracted nursing agency personnel and physicians. This also includes setting up Hospice patients in the hospital if the hospice staff is unable to make the in hospital visit.

What your background should be: The Clinical Infusion Coordinator shall possess a two year degree in nursing from an accredited or approved school plus two years of experience as a RN. Home hor Hospital Nurse experience preferred. CRNI certification preferred. Excellent interpersonal, verbal and written communication skills, time management and organizational skills, critical thinking skills, and flexibility are required.
