What you'll be doing
- We are searching for a dynamic Intake Case Manager, who will be responsible for: - Working with the newly referred hospital residents in order to receive initial documentation and, if required, helping them to receive documentation from Hospitals. - Making home visits to ensure initial program participation. - Providing help with finding, introducing and engaging individuals referred by the Care Coordination, Lead Health Homes and VillageCare services - Highlighting community / neighborhood services that are accessible for the hospital resident. - Keeping updated electronic tracking of every assigned individual and submitting completed documentation in accordance with the requirements of the program.

- We are on the lookout for an expert with the following background experience: - Sensitivity and awareness of all cultural and socioeconomic aspects of the community as well as its resources. - Bachelor
