What you'll be doing
- This office-based role manages the Transitions Team. The Manager will also work closely with process development, Health Service Directors and Medical Directors. The candidate is responsible for clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). Case management and coordination of care for members. Taking a lead role in setting direction and participating in or developing new programs. Setting team direction, resolving problems and providing guidance to members of the team. Ensuring team meets established performance metrics and performance guarantees. Adapting departmental plans and priorities to address business and operational challenges. Managing performance of professional employees and/or supervisors; up to 15 direct reports. Providing health education, coaching and treatment decision support for members.
What your background should be
- This position required current, unrestricted RN license in the State. Minimum of 3+ years experience as a manager and 3+ years experience in a hospital setting, acute care or direct care experience. Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment. Candidate must have a background in managed care, case management experience, Certified Case Manager (CCM), experience and exposure with discharge planning. Experience in utilization review, concurrent review and risk management. Strong organizational skills and multitasking abilities.

- Undergraduate degree or equivalent experience
