MH/SA Care Coordinator

What you'll be doing

The MH/SA care coordinator is responsible for assisting individuals with complex chronic mental health and/or substance abuse diagnoses in navigating the behavioral health system and to assure services in the least restrictive environment. The coordinator shall provide a person-centered approach to identify service needs and to link individuals to the most effective service and appropriate level of care and support, provide technical assistance to providers, assist in managing chronic illness, provide education about the system of care approaches in collaboration with stakeholders, monitor person-centered planning effectiveness and quality of care and ensure the use of evidence-based best practice service delivery methods. Job responsibilities may include working as a liaison in crisis centers, emergency departments and hospital systems.

What your background should be

At least two years of post-degree progressive experience providing similar services to the population served. Additionally, valid NC license or provisional license in profession (social work, counseling or psychology) is required. Additional training preferred.

Required Schooling / Training

Master degree in a human services field (such as psychology, social work, etc.)
