What you'll be doing
- Coordinates all clinical pharmacy functions for the critical care wards and acute care wards as assigned to help achieve positive patient centric outcomes through direct and indirect interactions with patient, providers and interdisciplinary teams. Attends and participates in multidisciplinary rounds, team meetings, conferences, and other meetings as required for the purpose of designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment. Works independently and effectively to assure appropriate, accurate drug reviews, and interprets prescriptions and physician orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness. Reviews patient medication profiles, contacts physicians and makes authorized changes in orders or prescriptions as required. Functions independently in day-to-day clinical and administrative duties associated with the position, consulting with higher level supervisors only when required. Demonstrates recognized expertise in the area of critical care and acute care management of patients. Monitors and assesses the outcome of drug therapies with advanced skill, including physical assessment and interpretation of laboratory and other diagnostic procedures. Works closely with physicians and nurses to monitor patient response to medications, including efficacy, safety, and outcome, with special attention to critical care. Submits summary data or documents a significant impact of interventions on pharmaceutical care as requested by pharmacy supervisor. Documents interventions and recommendations via CPRS progress notes and consults, demonstrating the ability to communicate orally and in writing to persuade and influence clinical and management decisions.
What your background should be
- Full, current and unrestricted license to practice pharmacy in a state, territory, commonwealth of the united states, or the district of Columbia. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for the areas of acute care and critical care. Expert knowledge of acute care and critical care activities. Advance skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Unofficial transcripts are required whenever qualifying based on education.
Required Schooling / Training
- Graduate of an accreditation council for pharmacy education accredited college or school of pharmacy with a bachelor degree in pharmacy and a doctor of pharmacy degree.
