What you'll be doing

Provide patient clinical coordination in a variety of environment including home, infusion suite, hospital, and doctor office. Train patient in the self-administration of treatment and how to quickly recognize and respond to symptom of adverse physical reaction to treatment. Coordinate with pharmacist to address change in patient condition or drug management issue. Collaborate and confer with branch staff regarding patient status and treatment need. Document all interaction and communication with patient, plan of treatment, assessment, treatment, patient response, and reaction to treatment, status of disease process and training activity into electronic device in a timely manner. Perform all other assigned task as required.

What your background should be

Minimum 2 to 5 years of relevant working experience; at least one year as a specialized registered nurse in acute or home care, home infusion environment; completion of intravenous skill inventory. Ability to lift and perform repetitive motion. Critical care experience preferred. Strong venous access skills required. Previous home infusion experiences a strongly preferred.
