Registered Nurse/Case Manager (PRN)

What you'll be doing
- Responsible to travel in the community to provide nursing assessments and to supervise and evaluate patients home. Responsible to open new request for service, developing plans of care with the patient and or family, and completing agreements for service. Provide visits and ongoing supervision to all patients according to physician orders and within required periods. Responsible for completing physician orders and comprehensive (OASIS) assessments within the periods dictated by the regulating bodies. Responsible to provide education and resources to patients and families within the community. Maintain confidentiality and adhere to HIPAA regulations on all clients. Attend in-services as scheduled.
What your background should be
- Minimum 1 year of prior experience is required. Medicare homecare experience is a plus. Valid RN license in Ohio; current Ohio driving license. Ability to organize a large quantity of information. Knowledge of Medicare and Medicaid regulations. Knowledge and adherence to HIPPA regulations. Excellent communication skills. Ability to work as a team member. Excellent organizational and time management skills. Ability to be flexible in schedule and to work in a stressful environment.
