What you'll be doing

The RN care coordinator is responsible to support clinical team in a medical home model approach. The candidate will teach patients with chronic diseases how to interface with the health care community and to grow in self-management and strategies. S/he will use motivational interviewing to have the patient truly engage in his or her own health. The incumbent will use effective communication and problem solving skills on a daily basis to provide quality patient care. Coordinate resources and work in a collegial relationship with elaborate health professional team. This position will provide proper documentation and reporting on all patient interactions using care management plus to manage patient panel. The role will collaborate with healthcare team to develop toolkits and plans of care for patients and families to promote wellness and self-management.

What your background should be

The candidate should have 3 years of experience as a registered nurse. Experience in patient education, case management, and/or health promotion desired. S/he should have experience with motivational interviewing a plus. PCPCH knowledge or interest a must. Experience working with underserved populations is desired, but not required. EMR experience helpful. Open to giving new ideas for improvement and receiving feedback to exist in an environment that is constantly seeking improvement. Interest in team-based program development and process improvement required. Possession of current Oregon RN license in good standing. Possession of current CPR certification.
