What you'll be doing

Provide culturally sensitive; solution focus; strength base; trauma inform individual, group, family, case management and in home therapy service. Participate with multidisciplinary team members in carrying out the goal and objective of each student individualize education plan (IEP). Utilize evidence based, best practice standard in the provision of care. Work in collaboration with members of the multidisciplinary team to conduct FBA and create BIP. Efficiently manage caseload, develop and document, clear progress note, case note, clinical formulation, DSM diagnosis, crisis prevention plan, round review and clinical psychosocial assessment. Complete all chart documentation as stipulated by agency, state and federal regulatory guideline and within compliance standard. Document all client care and communication as defined by agency, complete documentation in a well organize, timely and accurate manner. Coordinate students ongoing psychiatric and psychological evaluation. Attend all required meeting, to include PPT, planning meeting, other relevant meeting and conference throughout the state; also include weekly clinical supervision, model consultation session, round, treatment team and staff meeting. Proactively seek professional guidance, additional supervision and support from director of clinical service and demonstrate follow through with supervisory dictate. Meet documentation, service productivity expectation and target outcome as defined by agency; program; model, funder; insurance provider and regulatory requirement. Collect and maintain performance outcome data as stipulated by agency, model developer or other stakeholder.

What your background should be

Three to five years of clinical experience working with children, adolescents and families with sufficient experience, training and competency in the provision of culturally sensitive, strength based, trauma informed individual, group and family psychotherapy. LMFT, MSW or LCSW with CT 071 school social worker certification; or MFT with CT MFT school educator certification. Proficient understanding and knowledge of local and national best practice standards. Valid Connecticut driving license with good driving record. Possess highly developed, competent skills in family, group, case management and individual therapy. Experience working in an interdisciplinary environment (i.e., juvenile justice, education, medical and/or mental health profession).
