What you'll be doing
- Team Leader will manage daily operations of the home health care professionals that make up the project team and make sure the continuity of the patient care through out the day by managing project group's schedule. The person will manage patient and physician phone calls and communication through out the day. Team Leader will be chargeable for coordinating each patient's treatment plan by contacting community service organizations and assigned staff for particular patient cases. She/he will conduct medical chart audits, assure completion of record reviews of each patient for recertification and ensure financial compliance. Additional duties involves direct patient care by reconsidering and offering supervision for each patient's plan, assisting with employee training, taking daily staff reports on patient case loads, following up on patient lab work.
What your background should be
- 1 year of nursing experience
Required Schooling / Training
- ASN degree
